- Record USA registration number of the truck.
- Record month-day-hour (MDH) code and date of manufacture as found on the Vehicle Identi-
fication Data Plate.
- Record MILSTRIP document number, and the address of the unit receiving disposition instruc-
tions and/or replacement warranty part(s).
- Overseas units must clearly record their DODAAC number. Failure to enter this number will
seriously delay overseas shipments.
2. PREPARATION OF DA FORM 2402.
Use of DA Form 2402. The DA Form 2402 must Lx filled out and attached to any and all defective part(s)

removed from the vehicle. Refer to Figure 2 for an example of the completed DA Form 2402.
Completing DA Form 2402. Fill out the form as follows.

Block 1. Enter name of the Support Agency where the part is to Lx exchanged.

Block 2. Enter current date.
(2)
Blocks 3, 11, 15. Enter name of the organization turning in the part(s).
(3)
Blocks 4, 12, 16, 20. Enter NSN of the defective part(s).

Blocks 5 and 19. Enter noun nomenclature of the part.

(6) Block 6. Check the EIR block, if an EIR is being submitted. Check the EXCHANGE block, if a direct
exchange is required.
Blocks 8, 13, 17, 21. Enter nomenclature and manufacturer of the defective part(s).

Block 9. Enter model of the vehicle.

Blocks 10, 14, 19, 20. Enter serial number of the vehicle.

Section VIII. PREPARATION OF VEHICLE STATUS REPORT
1. PURPOSE. A Vehicle Status Report must be partially prepared for each newly delivered vehicle placed in Government
storage and completed when each vehicle is removed from storage and placed in service (refer to Appendix A). The prepara-
tion of this report is an important aspect of vehicle warranty terms between the Government and AM General Corporation.
It must be prepared properly and submitted in a timely manner (within 20 days from receipt of the vehicle) so that the
Government can fully realize the intended warranty benefits.
2. USE OF VEHICLE STATUS REPORT. AM General will provide blank report forms to the Government representative
at their plant. The Government representative will place four blank report forms with each vehicle scheduled for shipment
from AM General to Government storage. The forms will be used as follows:
