INSTRUCTIONS
1. Complete parts I and II when the vehicle is placed in storage. Distribute one copy to manufacturer; one copy to
vehicle; one copy to depot file; and one copy to TACOM, ATTN: DRCPM-HTL.
2. Complete part III of the form when the vehicle is removed from storage. Distribute one copy to manufacturer; one copy
to vehicle; one copy to TACOM, ATTN: DRSTA-MS; and one copy to TACOM, ATTN: DRCPM-HTL.
3. Forward the manufacturers copy to the Theurer Greenville Corp. Southwest Truck Body Co. or, Heller Truck Body Co.
(depending on the builder indicated on vehicle data plate) and forward to the address indicated in paragraph 1-1.
4. Distribute copies within 20 days of the day the vehicle is placed in or removed from storage.
